Health Risk Appraisal Assessment Critique. The Rapid Eating Assessment for Patients (REAP) tool is a brief validated questionnaire that helps the provider to perform diet and physical activity assessment. It facilitates nutrition assessment and counseling patients due to their answers in the provider’s offices (Miller, 2015). A poor diet plays an essential role in health problems such as cancer, stroke, and diabetes mellitus types two, among others. The changes in the diet can help prevent such diseases and reduce medication costs. Therefore, through the REAP tool, patients can understand their health well and the effects of their diet.

Patients conduct the Self-Management Tool as a way of monitoring their health issues outside formal institutions. It provided all the informative content the patient requires for self-assessment. They help the patient to keep track of their health issue and receive oversite relief as part of their NBCH Health Plan (Leone, Rosanna, & Peter, 2018). Unlike the REAP, where the answers provided by the patient must be evaluated by the provider, the tool allows the patient to make their own decision without expecting a question for their decision.

The REAP tool provides guidance and counseling to a patient to help them understand the consequences of their behavior, and they might consider changing their decision on the diet. However, the self-management tool does not provide such guidance, and patient health might be affected as they have no guidance to make the best decision Both tools are designed to help patients keep track of their help. From the assessment, a patient can identify their areas of weakness and poor diets. Through this assessment, they can make a choice that helps them improve their health. Both the tools connect the lifestyle habit to risk factors for various diseases and help the patient keep a truck f their diet.

Advantage of using REAP tool
The tool helps an individual understand their current health status by evaluating their various conditions. Also, it enables them to change their health behaviors. During the assessment, patients are provided with an assessment sheet of their diet, and after asking their decision, a provider guides them based on their decisions. Besides, it helps the patient understand the effects of their diet and the effects it can use to work. The tool helps the patient to make the best decision for their health as they receive counseling later.

Disadvantages of the REAP Tool
The tool allows the patient to disclose their personal information, which many people feel uncomfortable. In some instances, not all people can afford to have a balanced diet meal, and the tool requires patients to have a good diet for their health. Therefore, those people who cannot afford such a lifestyle may feel uncomfortable providing such details.

Advantages of Self-Management Tool
The tool provides the privacy of the information provided by the patient. Once a patient carries the assessment, the data is uploaded to the system, and feedback provided to the patient without the provider seeing them. Therefore, they can disclose any information as it feels secure to get feedback from the system (Leenen et al., 2016)1. Also, the tool gives confidence to the patient as no one is going to ask them why they made particular decisions.

Disadvantages of the Tool
The patient might lie about their condition as the provider cannot see them physically. Such a condition affects their health. Also, providing the patients with these tools is expensive as compared to them visiting the hospital.

Health Risk Appraisal Assessment Critique, the REAP tool can best be utilized for all patients who can receive their medication and do not require the guidance of the nurses. Also, those patients with obesity are treated under this tool. The forms are be filled immediately; the patient enters the hospital to guide the provider on their problem. The self-assessment tool can be utilized by those patients who are treated from home due to various conditions, especially Epilepsy. The nurses can help them with the assessment.
